Wegovy Side Effects

Wegovy titrates to 2.4 mg — the highest semaglutide dose in routine use — through five fixed steps. Which means the side-effect story here is really a story about escalation, and about the one decision you get to make five times.

What is different about Wegovy

It climbs higher. Wegovy titrates to 2.4 mg — the highest semaglutide dose in routine use, and meaningfully above where most people on Ozempic sit. More drug means more of everything, including the gastrointestinal effects.

It climbs in five fixed steps. Which reshapes the whole experience. Side effects on Wegovy do not accumulate in a steady line; they arrive in five distinct waves, each following an increase, each settling over about a week. The month between increases is usually the easy part.

That structure means the side-effect question is really a titration question, asked five times: go up, or hold?

The loss is faster, which brings its own problems. Wegovy is prescribed for weight management, and mean loss in STEP 1 was around 15% at 68 weeks. Rapid weight loss from any cause carries consequences that are not drug effects at all but arrive on the drug’s schedule — gallstones, and the hair shedding that turns up around month four.

No hypoglycemia complication in most people. Unlike Ozempic’s typical patient, most people on Wegovy are not also taking insulin or a sulfonylurea, so that risk is largely absent. If you are, the same warning applies.

The five waves

Each escalation produces roughly the same shape: onset within 24 hours, peak around day two or three, resolution over about a week. That timing tracks the concentration curve — blood levels peak one to three days after injection, not immediately.

Knowing this in advance is most of what makes it tolerable. Almost every person I have seen quit in month two quit during an ordinary, expected, self-limiting week that nobody had warned them about.

The consequences of losing weight quickly

Two things that get blamed on the drug and are really about the rate.

Gallbladder disease. Rapid weight loss increases gallstone risk regardless of method, and there is a gallbladder signal in the GLP-1 data. New right-upper-quadrant pain, especially after fatty meals, with or without fever or jaundice, needs evaluation promptly.

Hair shedding. Usually telogen effluvium — a delayed, diffuse, temporary shed triggered by the deficit rather than by the molecule. It starts two to four months after the trigger, which is why it feels like it came from nowhere, and regrows over six to nine months. Protein, avoiding an extreme deficit, and checking ferritin are the interventions that help.

The red flags

Not tolerability problems. These belong on a phone, not in a plan.

  • Severe, persistent abdominal pain radiating to the back, often with vomiting — possible pancreatitis. Stop and seek care.
  • Right-upper-quadrant pain, fever, or jaundice — possible gallbladder disease.
  • Vomiting or diarrhoea you cannot keep ahead of — dehydration and kidney injury.
  • Any sedation or procedure — tell the anaesthetist, unprompted, in pre-op.

Managing the five waves

Slow the titration. This is the lever, and it is under-used. The label explicitly permits delaying escalation if a dose is not tolerated. Eight or twelve weeks at a step instead of four is normal, and there is no prize for reaching 2.4 mg on schedule. I would far rather someone arrive at 1.7 mg comfortably and stay there than reach 2.4 mg and quit.

Eat for the window. Smaller meals, less fat, nothing fried, in the two to three days after the shot. Stop at first fullness.

Get ahead of constipation rather than waiting it out.

Protect protein. 1.2–1.6 g per kg per day. This is not a side-effect measure in the obvious sense, but under-eating protein produces the fatigue, the hair shedding, and much of the misery that gets attributed to the drug. I have argued this at length in Muscle Is the Whole Game.

Know your own pattern before the next increase. This is the part that generalises least well from an article and most from a record.

The reason that matters specifically on Wegovy is the escalation structure. You face the same decision five times, and each time the useful evidence is how the last increase actually went — not how you remember it going four weeks later, in a fifteen-minute appointment, when memory has smoothed it. Arriving at that conversation with the real pattern is what turns “it was fine, I think” into a decision.

To be clear about the boundary: it manages tolerability. It does not replace your prescriber, and nothing on the red-flag list above should ever be routed through an app.

The three sentences

  1. If the side effects are bad, the answer is usually a slower titration — call before you quit.
  2. Severe persistent abdominal pain, or vomiting you cannot stay ahead of, means same-day contact.
  3. Tell every clinician you see, including your surgeon and anaesthetist, that you take this.

Questions I get about this month

What are the most common Wegovy side effects?
Nausea, diarrhoea, vomiting, constipation, abdominal pain, headache and fatigue were the most frequently reported in the STEP trials, and gastrointestinal effects were the most common overall. They were generally mild to moderate and transient, clustering around dose escalations and settling within roughly a week at a stable dose. Constipation and reflux are under-discussed relative to how often they drive people to stop.
How do I reduce Wegovy side effects during a dose increase?
The most effective single measure is slowing the titration, which the label explicitly permits when a dose is not tolerated — staying eight or twelve weeks at a step instead of four is a normal clinical decision. Beyond that: smaller meals, less fat and nothing fried in the two to three days after the injection, stopping at the first sign of fullness rather than finishing the plate, and treating constipation early with fibre, fluid and an osmotic laxative if needed.
Does Wegovy cause gallbladder problems?
Rapid weight loss from any cause increases the risk of gallstones, and there is a gallbladder signal in the GLP-1 data specifically. The practical implication is symptom awareness rather than avoidance: new right-upper-quadrant pain, particularly after fatty meals and especially with fever or jaundice, needs evaluation promptly rather than at your next routine appointment.
Does Wegovy cause hair loss?
Shedding that appears around months three to five is usually telogen effluvium — a delayed, temporary, diffuse shed triggered by rapid weight loss, reduced energy and protein intake, rather than a drug effect on the hair follicle. It typically begins two to four months after the trigger and regrows over six to nine months once intake stabilises. Hitting your protein target, avoiding an extreme deficit and checking ferritin are the interventions with the best rationale.
